Office Policies

 

Appointment

We attempt to schedule appointments at your convenience and when time is available. Since appointed times are reserved exclusively for each patient, we ask that you please notify our office 48 hours in advance of your scheduled appointment time if you are unable to keep your appointment. Patients who are scheduled for 2 hours or more are asked to give 1 week notice if possible so that other patients can be scheduled in your time slot. We realize that emergencies occur, but we ask for your assistance in this regard. 


Insurance

Our office does not accept insurance as a form of payment. We do not have a contract with your insurance company, only you do. Therefore, we are not responsible for how your insurance company handles its claims, or for what benefits they pay on a claim.  We will file your insurance claim as a courtesy and your insurance company will pay you directly.

Our office accepts Visa, MasterCard,Discover and American Express credit cards. Payment can also be made by cash, check, or money order.

As a service to our patients, we are pleased to offer the CareCredit card and Lending Club.
